Home Health Aide Home Health Aides (HHAs) are a necessary part in allowing you to recover and heal quickly, while at the same time keeping your surroundings in good order. Nina's Health Care Services, LLC is proud to have dedicated Home Health Aides in our team to provide care and support for both you and your family in times of poor health. While Home Health Aides generally can take over the household chores while you or your loved one continue to recuperate, they can also provide personal assistance if needed. Under the direction of a case manager, our home health aides can provide assistance with:

• Bathing and other personal hygiene (if needed)
• Personal Care or Bed-Side Care (if needed)
• Health Monitoring and Documentation
• Mobility or Ambulation Support
• Incidental and Schedule Transportation
• Grocery shopping
• Light Meal Preparation specified by client's recommended diet
• Light Housekeeping

Council of Aging is State of Ohio Medicaid Program that allows Nina's Health Care Services, LLC to provide care at home for seniors as an alternative to nursing home care. Senior services available include homemaker, transportation, personal care services, home-delivered meals, emergency response systems and adult day services.

To be eligible, a person must be 60 or older, meet financial requirements, and have qualifying functional impairments (health problems, etc.). In addition, those who are eligible for the program receive a Medicaid card to cover the cost of medications and health care. Mental Retardation and Developmental Disabilities – MR/DD

The principles of Self Determination are the foundation upon which Nina's Health Care Services, LLC establishes policies, procedures and support opportunities for eligible children and adults. The Agency has organized its Support Services across four developmental stages:

• Early Intervention
• Family Support
• Transitions Supports
• Adult supports

The transition between each support stage is an important milestone in the development of each person. They mark a significant change in that is responsible to provide services and how services are delivered. In each stage the services are coordinated and managed by a professional referred to as a Support Administrator, who is the single point of contact for the client or their family. Support Administrators help the client, their family, and their team to navigate available resources and service delivery.

Our support system focuses on issues of informed consent, dignity of risk, health and safety skills, increasing independence, and ability to speak and decide for one 's self. Supports assist youth leaving the structure and services of the school system to develop skills needed to take on adult responsibilities such as work, adult education and daily living skills. Adult supports work to maximize available resources and developing opportunities for adults to gain self-dependence, participate in their community and live as independently as possible in their own home. Our Supports also address the needs of adults with disabilities growing older with more healthcare needs to maintain independence and the skills they have developed.

The majority of older adults prefer to remain in their own homes as they grow older. With assistance from family caregivers and/or private duty nursing and nurse aids, they need never to move to a senior living community. This is a positive choice for those who wish to remain close to their history, their possessions and their family memories. Regardless, everyone needs companionship to avoid feelings of isolation and loneliness. Living alone is a reality for many seniors. For some, the isolation can lead to depression and increase the risk of dementia. Nina's Health Care Services, LLC' Companion care provides seniors with a friend and companion to share activities, conversation, recreation, and other daily activities. Our Companion care can also provide a range of other duties and responsibilities that make the lives of seniors easier.

• Assistance with activities of daily living (ADLs) including grooming, dressing/undressing, toileting, bathing, and eating
• Assistance with transportation to shopping, doctor's appointments, restaurants, and other activities
• Light house-keeping duties such as vacuuming, dusting, cleaning, etc.
• Meal preparation and cooking with the nutritional needs of the senior in mind
• Medication management or assisting with administering and refilling prescriptions
• Memory care for those with Alzheimer's/Dementia
• Sharing range of recreational activities including walks, swimming, or your choice of activity.
• Assistance when there is a crisis

• Pet care including feeding, walking, clean up and Vet visits
• Act as a liaison with families who live away
